BURGHAUSEN, GERMANY, April 8, 2020 / — The various therapies for the treatment of cancer have significantly improved in recent years through intensive research and modern technologies and immunological research.

Nevertheless, also modern methods such as radiotherapy and chemotherapy are reaching their limits due to their damaging effect on healthy tissue. In these cases, innovative therapies such as the intra-arterial infusion- and isolated perfusion chemotherapy are a promising option. The results of a recent study in the Journal of Cancer Research and Clinical Oncology, which examines the regional intra-arterial infusion chemotherapy as a therapeutic option in patients with head-and-neck tumours, show that this method leads to a more efficient tumour control with prolonged survival at maintained quality of life with significantly fewer side effects compared to standard therapies: In none of the 97 patients a tracheostomy or tube feeding had to be performed and the swallowing function remained unimpaired in all cases. It therefore represents a promising option – especially against the background of current demands for de-intensification of therapies for head and neck cancer: Since especially the serious side effects of radiotherapy such as speech and hearing loss, painful swallowing disorders and permanent inflammation of the mucous membranes are associated with a considerable limitation of the quality of life and lead to suicide rates that are significantly higher than in any other type of cancer, as evidenced by US and Canadian studies.
Good response rates in studies of regional chemotherapy include ovarian, pancreatic and liver tumours.

Recent study on advanced ovarian cancer: Isolated perfusion therapy with chemofiltration leads to rapid remissions with maintained and improved quality of life

BURGHAUSEN, GERMANY, April 8, 2020 / — Ovarian tumours are among the most aggressive cancers in women and rank fifth in all gynaecological cancers. The disease is often only recognised very late and is then hardly controllable. Only about one third of the patients survive the five years after the initial diagnosis. If metastases have already formed in the abdomen, often only a radical removal of various organs and organ parts with subsequent chemotherapy is the only thing that can be done, which is accompanied by serious side effects and severely affects the quality of life. A current study at the oncological specialist hospital Medias-Klinikum in Burghausen shows that quality of life and survival time can be improved significantly with a special, regionally limited, highly concentrated chemotherapy.

Each year, about 9,600 women in Germany develop ovarian cancer, 5,500 of which die each year. Hence, this tumour is the main cause of death for all cancers in women. The main reason for the high mortality rate is that in about 70 percent of the cases the disease is only recognised in late stages, as it is largely symptom-free until then. Often, metastases have already formed in the peritoneum and in organs such as liver, pancreas or spleen. Survival prognosis is poor in these cases and often is only a few months, depending on the stage at the diagnosis. Furthermore, metastases in the peritoneum lead to large fluid accumulations in the abdomen, the ascites, which can be associated with severe pain. Standard therapy provides for the tumour to be removed as completely as possible with subsequent chemotherapy. Despite therapy, however, new tumours develop in almost half of the patients within two years that often cannot be operated on and show resistance to a repeated chemotherapy. "Such resistance can usually only be broken through by an extremely high concentration of chemotherapeutics", explains Prof. Dr. Karl Aigner, Medical Director of Medias-Klinikum in Burghausen, and adds: “However, this is not possible with chemotherapy, which is carried out via a venous access and the entire blood circulation as standard. The extremely high concentration would also massively damage the patient's healthy tissue.” This is exactly where a special treatment modality is available that was examined in a study at Medias-Klinikum in patients with inoperable and chemo-resistant tumours.

Local limitation of chemotherapy reduces side effects
The principle of isolated "Hypoxic Abdominal Perfusion Chemotherapy" (HAP) is a locally or regionally limited administration of highly concentrated chemotherapy with subsequent removal of the drugs from the blood in order to avoid toxicity. "In this way, we can achieve much higher concentrations of chemotherapeutics in the tumour and metastases than would be possible with systemic chemotherapy, and can overcome drug resistance in most cases," explains Prof. Aigner, who developed the method. For this purpose, the blood supply to the region affected by the tumour and metastases is briefly disconnected from the rest of the circulation and flooded with a high concentration of chemotherapeutic drugs. After an exposure time of about 15 minutes, the drugs are removed from the isolated region by means of a special filtration process and the entire blood circulation is restored thereafter. Because of the locoregional administration of chemotherapy with subsequent chemofiltration, patients experience only few to no side effects such as bone marrow suppression, reduced performance or hair loss.

Promising study results
At Medias-Klinikum, this treatment was performed in 45 patients with inoperable, chemo-resistant tumours of the ovaries in late stages, metastases in the peritoneum and in various organs, as well as distinctive ascites. All patients had previously undergone several standard therapies in other hospitals. On the basis the tumour marker CA 12-5, it could be shown that more than 70 percent of the tumours responded well to the therapy. In more than 70 percent of patients, the partly massive accumulation of fluid in the abdomen (ascites) disappeared or could be significantly reduced already after two regional chemotherapies. Median survival time across all tumour stages, which was just below 12 months, was about twice as high as in other studies. All patients were asked about various quality of life parameters such as diarrhoea and nausea, hair loss, tiredness and fatigue, etc., which are common among patients treated with systemic chemotherapy. It turned out that regionally administered chemotherapy with subsequent filtration is clearly superior to standard therapy. "Overall, these study results are promising and make it clear that HAP is at least a therapeutic option for patients in late tumour stages," Prof. Aigner comments the study and adds: "We have patients who have been symptom-free for more than 13 years after treatment and are without evidence of tumour."

This has caused major disruptions to everyone’s daily life and most of the world is confined to their homes in a strict quarantine with the economies of most countries having come to a standstill. Many people around the world have lost their jobs as businesses shut down to help slow the spread of the virus with only “essential services” continuing to operate.

It’s a dark and difficult time for everyone but as long as we follow protocol and lie low for a few months, it’s one that will hopefully be behind us before we know it.

Self-quarantine is the only way to effectively curb pandemics like COVID-19 and much of the world is confined to their homes until further notice. Quarantine can be tough and lonely but that’s not to say there isn’t a silver lining to it.

Bard on the Beach Shakespeare Festival Announces 2020 Season Cancellation Due to COVID-19 Concerns

VANCOUVER, BC, CANADA, April 7, 2020 / — Western Canada’s largest Shakespeare festival has announced it will not be presenting a 2020 season this summer. The Bard on the Beach Shakespeare Festival was scheduled to run from June 10 to September 26 in Vancouver’s oceanside Vanier Park, where the signature annual event has been staged for the past thirty years. However due to the risks and unpredictability of the COVID-19 pandemic, the Festival has now decided to cancel its entire 31st Season.

“Our team has spent the past month in an intensive exploration of what might be possible for our season, given the impact of COVID-19” said Founding Artistic Director Christopher Gaze and Executive Director Claire Sakaki in a joint statement. “Our first focus throughout has been the safety and well-being of our patrons, our staff and volunteers, our donors, sponsors and partners, and the greater community we serve and entertain. It has now become clear to us that if we stage a Festival in the Park this summer, we won’t be able to ensure the health and well-being of all those people who would gather together at our site and in turn, be in contact with others. And so, we have made the deeply painful decision to cancel our Season. We know the impact it will have on so many people we care about – and we are heartsick about that – but we are sure that we are making the responsible and necessary choice.”

The Festival’s 2020 lineup featured Shakespeare’s beloved comedy A Midsummer Night’s Dream, playing in repertory with his epic historical drama, Henry V, both on the BMO Mainstage. The Howard Family Stage in the Douglas Campbell Theatre was to host two contrasting productions: Love’s Labour’s Lost, a restaging of Bard’s 2015 hit Jazz Age production, and Paradise Lost, Erin Shields’ critically acclaimed modern take on the battle between good and evil. The Season also included a variety of special events, from orchestral and opera concerts to the popular Bard-B-Q & Fireworks nights.

Says Gaze in a video statement, “There is some good news in that we are able to confirm we intend to present the same dynamic 2020 lineup of plays in our 2021 Season. Each production’s concept is unique and innovative, and we’re thrilled that the promise of each of them will survive and be fulfilled next summer.” Some of the country’s most experienced theatre-makers have been working on preparing the productions and Gaze has confirmed that the Festival intends to reassemble the same teams a year from now.

Over three decades, Bard on the Beach has grown to be a signature event on Vancouver’s summer entertainment and tourism calendar. The not-for-profit festival draws over 100,000 patrons each season, including many thousands of young people who attend subsidized School Matinee performances. Over two-thirds of Bard’s eight-million-dollar annual operating budget comes from ticket and ancillary sales, generated over its four-month run, and the organization annually employs almost 300 administrative and production staff and artists. Says Sakaki “We know that the cancellation of our full slate of over 200 performances will have a significant financial impact on the livelihoods of the people who rely on us for a large portion of their annual income – as well as on our organization. We are deeply troubled by those facts. But we believe we must make the decision to cancel – and make it now – for the safety of all and for the longer-term financial health of our company.”
